With two distinct neighborhoods, Sierra continues to meet the complex medical and behavioral needs of the community. prasAda, Piñon Management’s innovative holistic mental health program, is Sierra’s neighborhood approach to caring for persons with behavioral needs in a secured, calm setting. sarvatA is the non-secured neighborhood where we care for the mind, body and soul within a creative environment, meeting the needs of a diverse population.

Current Events

Overview: Sierra has 83 licensed beds and is located north of 6th Avenue off of Sheridan and Colfax. We serve geriatrics, younger adults, non-wandering dementia, behavioral, mentally ill, 27–10 certified, Cerebral Palsy, medically complex care, HIV⁄AIDS, neurological disorders, strokes, diabetic care, alcohol dementia, wound care, hospice care and respite care.

Specialized Clinical Services: 24-hour nursing care; Physical, occupational, speech and respiratory therapies; Trach care; Wound care; G-tube; IV therapy; Tube feeding; PCA pump; PICC line; Secured behavioral unit; Mental health programming; Non-secure dementia care; Restorative therapy

Enhanced Care Services: Resident council; Transportation; Laundry and housekeeping services; Code Alert; Beauty and barber services; Culturally-diverse activities programming; Individualized behavioral programming

On our secured neighborhood, The prasadA Community, members are encouraged to participate in a variety of life enhancement, work therapy and cultural recreational activities in order to facilitate a transition to a less–restricted environment and ultimately back into the surrounding community.

Encouraging resident and family members to decorate and furnish their rooms with personal belongings is what makes our home become your home!

Sierra Healthcare Community is partnered with It’s Never 2 Late, a computer program that links young adults and elders in long-term care communities with the world! Special adaptive devices are available to ensure all can participate. This unique program allows residents to surf the internet, play games and stay connected to loved ones through email. The possibilities are endless!

Staff and community member interaction is what makes Sierra unique! Activities are driven by the residents.
